DSH Bonus

DSH – Hospitals serving a large population of indigent patients may receive a DSH bonus to their Medicare payments.  This Medicare DSH payment is calculated in the Medicare cost report of the hospital.  This is summarized by CMS in the following summary.


Hospital Outliers

Hospitals can receive bonus payments above the DRG payment for outliers.  Outliers are payments for very expensive cases.  A CMS summary of this can be found here.


Hospice Caps

There is a cap on Medicare reimbursement for inpatient hospice care days, and also a cap for an average payment per beneficiary.  Palmetto’s website has a nice summary of these caps and a calculator to calculate overpayments due to these caps. It can be found here.


Medicare GME and IME Credit

Although the hospital Medicare cost report computes a bonus for GME and IME for Medicare HMO days, the hospital must send no-pay bills to traditional Medicare for those days. (PRRB 2009-D20).


New Outlier Computations for HHA in 2010

HHA Medicare payments in 2010 will have outliers computed based upon a Fixed Dollar Loss (“FDL”) of .67, compared to an FDL of .89 in 2009.  The FDL is adjusted for the wage index of the HHA’s location.  (The HHA Medicare cost report is used to compute the outlier payment.)


HHA 2010 Medicare Payment Update

HHA 2010 Medicare payment update has a 2.0 inflation update, but 2.75% reduction for ‘unwarranted’ case mix increase, and changes in outlier payments (10% cap per agency). The Federal Register shows an increase of the base rate for 60-day episodes from $2271.92 to $2312.94 (1.8% increase).

Influenza Vaccine Payment Allowances – Annual Update for 2009-2010 Season

The flu vaccine payment allowance has been updated for the current year.  The final reimbursement for the vaccine is settled on the Medicare cost report.  More information can be found here.


Reflections from Fall Medicare Cost Report Training

Fall has definitely arrived!  The weather is getting cooler and leaves are changing colors.  The kids are back in school and are busy learning.  We here at Progressive Provider Services have also been busy with our ‘Back to School’ Fall Medicare Cost Report Seminars.

Solomon Melamed hosted Understanding the Medicare Cost Report for Hospitals.  This well attended seminar gave the participants an in-depth analysis of all the detailed components necessary to complete a Cost Report for a hospital.

Blimi Schwartz hosted Medicare Cost Reporting 101 for Skilled Nursing Facilities (SNF).  This webinar clarified and clearly explained the Cost Report and participants left this informative webinar with a confidence of the various aspects of the Cost Report.

Rachel Vaks was the host for Understanding the Medicare Cost Report for Home Health Agencies (HHA).  Participants were given a background to Medicare and an overview of the Cost Report process.  Then, participants were walked through the actual Cost Report together with the PS&R and given a clear understanding of the many components of the Cost Report.
