There is a new worksheet that is required on the SNF Medicare Cost Report,  form CMS 2540-10.  The new worksheet is being called S-3 Part V and is required for cost reporting periods beginning on or after January 1, 2012.

This worksheet requires Skilled Nursing Facilities (SNF) to separately report expenditures for wages, benefits, and hours  for direct care staff, breaking out (at a minimum) registered nurses, licensed professional nurses, certified nurse assistants, and therapy staff.  The direct salaries information is entered on lines 1-13, column 1  and the contract expense information is entered on lines 14 – 26, column 1.  Column 2 allows input for employee fringe benefits (including payroll taxes). Column 3 yields a subtotal of Column 1 plus 2.  Column 4 allows input for hours paid. Column 5 then computes an average hourly wage for each line.

Per the CMS final rule published November 9, 2012, any SNF Medicare bad debt not claimed with the 2012 Medicare cost report will only be reimbursable at 65% for private pays and 88% for copays of dual eligibles (those who also have Medicaid eligibility). This 88% percent will be further reduced in the next two years until it is phased in to only 65%.   This rule can be found at the 2012 Federal Register , pages 67518 and 67519. Page 67519 has a clear chart showing the reductions.

There were a lot of changes recently put out by CMS for the Skilled Nursing Facilities Medicare Cost Report, the SNF 2540 form.   So many, in fact, that the name of the form has changed from 2540-96 to the 2540-10.  These changes are all in effect for cost reports beginning 12/1/10.

The following is a highlight of the major changes:

  • The cost center line numbers have changed.
  • Form 339 is now part of the Cost Report.
  • Subscripted lines have been removed.
  • Intern and Resident have been replaced by Nursing and Allied Health Education.
  • The Provider Number is now known as the Provider CCN (CMS Certification Number).
  • In addition, there have been many changes to the Home Health Sub Worksheets.
